HOW TO OBTAIN SPECIMEN PERMITS

The first step in completing the application to collect specimens is to obtain permission from the management of the slaughterhouse to collect the materials you need. If you are not aware of a slaughter house in your area, call us at (919) 733-4136 and we will direct you to one. You may also use the link "Plant Directory" on the left side of this page to see a list of inspected establishments.

Once you have obtained permission from the slaughter house management, complete and submit an application to collect specimens. To do this you may either:

1. Fill out our electronic application form and submit it directly from the web site.

2. If you prefer, print off the hard copy form, Application to Obtain Specimens from Official Establishments, and fax or mail the completed document. Go to the Contact Information page for the fax number and mailing address.
