If you are obtaining a PIN in order to show at the N.C. State Fair or to participate in the Hay Relief Program, please attention this at the top of the premises registration form.  This will enable us to expedite your request.

This form is NOT intended to be submitted online. You MUST print the Premises Registration Form. The NCDA&CS, Veterinary Division requires a signature in order for your form to be processed. Failure to sign the form will result in your form being returned to you to be signed, thus delaying the assignment of your PINs.
